(a)  Registration Form. Complete the registration form for the Service by providing true, accurate, current, and complete information about yourself as prompted by the registration form. Note that any information that you provide which is untrue, inaccurate, or incomplete may disqualify you from eligibility to receive the Gift.

(b)  Survey & Special Offers. All survey questions and special offers are optional. You may either answer or skip all survey questions and special offers that are presented following your completion of the registration form. (c)  Gift Redemption. Once you receive the Gift Claim Form please follow the step by step instructions as stated on the Claim Form. Once your eligibility is confirmed, you will receive a Validation Form via Email that will contain instructions on receiving the Gift. You will receive the Gift within 8-10 weeks upon receipt of the Gift Claim Form.
